Investigation Summary
On November 10, 2011, Employee #1, a temporary worker for Sanyo Foods Corporation of America, was checking the operation of the Distributor Machine, where noodles are dispensed from a batch into package-sized portions. Employee #1 was positioned next to a kettle, where brine is stored and pumped into sprayers on the production line. The kettle, which does not have any automatic shut-off sensor, had overflowed. The concrete floor was slippery from the brine and residual oil. Employee #1 slipped on the floor when he started to walk away, and he grabbed the frame of the Distributor machine to keep from falling. An unguarded chain and sprocket was operating on the inside of the frame. The tips of four of his fingers were pulled into the pinch point between the chain and the sprocket. All four tips were amputated by the operating chain and sprocket. Emergency responders were called, and Employee #1 was taken to a local medical center for treatment.
