Investigation Summary
On October 24, 2011, Employee #1, a record press operator of Erika records Inc., was working with the press in full operation mode, when he attempted to make an adjustment by reaching approximately 16-18 inches through a 3-4 inch unguarded vertical opening in back of the press and placed his hand and arm into the point of operation. The act triggered an optical sensor activating the press die to close on his left hand and arm.
