Investigation Summary
On August 24, 2011, an employee was inspecting an area coworkers had recently cleaned. A Hyster forklift, company ID number 13179 driven by a coworker struck the employee. The forklift operator was about to place a load into its rack space and did not see the employee before striking her. The employee suffered crushing injury to her right foot, resulting in amputation of two toes. The employee was hospitalized and treated for her injuries.
