Employee's Finger Crushed and Amputated in Milling Machine

On June 11, 2011, Employee #1, a machine operator, was removing a chuck from a TAKISAWA computerized milling machine. Employee #1 dropped the chuck onto a moving scrap conveyor located below the point of operation inside the mill. The mill door interlocks were in the deenergized position; however, the scrap conveyor was in full operation. As Employee #1 grabbed the moving part, his left index finger tip was caught between the part and a metal machine component. Employee #1's left index finger was crushed and amputated at the first digit. Employee #1 was hospitalized and treated for his injuries.
