Investigation Summary
On July 28, 2011, Employee #1, a mechanic, was called to fix a stalled belt on a conveyor truck. While Employee #1 lifted the boom of the belt conveyor by using an extra belt conveyor to access the engine compartment. While Employee #1 was under the boom reaching into the engine compartment, the belt conveyor holding up the boom of the stalled belt conveyor rolled back. The boom fell on Employee #1, crushing and killing him.
