Investigation Summary
On April 4, 2011, Employee #1 and Employee #2 were pulling up the rubber membrane of the roof of a building that was in the process of being demolished. The rubber membrane was getting caught in the teeth on the bucket of the track hoe as the operator tore the building down. Employee #1 and Employee #2 had been on the roof for approximately two hours. When Employee #1 sat down on the skylight on the roof to rest, he broke through the skylight and fell to the concrete floor 18 ft 6 in. below. Employee #1 was killed in the fall.
