Investigation Summary
At approximately 9:00 a.m. on September 12, 2011, Employee #1 was operating a 20,000 PSI gun-type hydroblaster, and was spraying a furnace when the hose connected to the gun crimped. The resulting water blast hit Employee #1 and caused a 4-inch horizontal cut under Employee #1's right pectoral muscle. Employee #1 died 90 minutes later.
