Investigation Summary
On September 27, 2011, Employee #1 was in a store being remodeled, clearing an area to stack drywall sheets. He was electrocuted when he contacted an energized 480 volt wire that was protruding from a conduit on an adjacent wall. The conduit had been broken during the demolition of an existing wall ten days prior to the incident. The end of the wire had been capped with a wire nut, and the circuit breaker had been turned off; however, the breaker had not been locked out or secured.
