Investigation Summary
At approximately 1:30 p.m. on April 7, 2011, Employee #1, the foreman, was observing three employees install curtain windows on the 18th floor. The employees started to lose control of the window and one of the employees called for help. Employee #1 grabbed the window and was swept out the window opening. Employee #1 fell three stories and landed on the 15th floor back roof suffering unspecified injuries.
