Toxic Fumes Overcomes Employee

On February 18, 2012, Employee #1 discarded liquid waste water sludge from an overhead tank into a tanker truck. The driver of the truck found Employee #1 lying over the opening of the tanker and was unresponsive. Employee #1 died from asphyxia.
