Investigation Summary
On July 24, 2011, two employees and the owner of Silvio Rodriguez Dba Silvio Rodriguez were installing shingles on the roof of a new residential structure. They removed their personal fall arrest systems and performed the last hour of work with no fall protection. Employee #1 slipped while transferring from the steep roof to the ladder. Employee #1 fell approximately 19 feet to the ground. Employee #1 suffered one or more unspecified fractures and was hospitalized.
