Investigation Summary
On February 22, 2011, Employee #1 was discovered by a coworker unresponsive. Emergency services were contacted and, upon their arrival, Employee #1 was taken to a nearby medical facility, where she was determined dead. Upon further medical examination, it was determined that Employee #1 died from a heart attack.
