Investigation Summary
On March 28, 2011, Employee #1, a blower truck driver, was applying bark dust in the backyard of a private residence from a 5.5 ft wall. He climbed the wall because he had no foot traction on a steep hill. As Employee #1 applied the bark dust, he lost his balance and fell, fracturing his jaw. He was transported to a medical center, where he was hospitalized.
