Investigation Summary
On January 29, 2014, Employee #1, a truck driver, was picking up a loaded semi-trailer from a local transportation hub station. Once loaded, a switch truck is used to position the trailers for pick-up and parked them approximately 9 in. apart. At around 1:30 a.m., Employee #1 was observed backing his truck under a trailer. A few hours later, the tractor was observed to be connected to the trailer and running. It was believed he was sleeping. At approximately 5:00 a.m., a switcher driver was relocating another trailer and observed Employee #1 between the two trailers. Upon closer investigation, it was discovered that he was pinned and unresponsive. Emergency medical personnel were summoned. Emergency responders arrived and extracted him by using air bags between the trailers. Employee #1 was found dead with numerous fractured ribs, abrasions, and petechial hemorrhage to the eyes, lungs and face. The cause of death was asphyxiation. It is believed that the trailer king pin was not properly seated in the fifth-wheel and when the landing gear was raised, the trailer shifted enough to pin and kill Employee #1.
