Employee's Leg Is Lacerated When Run over by Forklift

At approximately 1:00 p.m. on May 3, 2011, the operator of a Komatsu electric industrial truck (forklift), with a weight capacity of 6,550 pounds, last observed Employee #1 on the right side of the forklift, approximately 2 feet to 3 feet away. The forklift operator stated that he looked over his left shoulder prior to backing up and did not observe Employee #1, a machine operator/helper. As the forklift operator repositioned the forklift and backed it up approximately 2 feet to 3 feet, the forklift struck and ran over Employee #1. Employee #1's left leg was pinned between the rubber tire and frame of the forklift, and emergency services had to free him. Employee #1's leg was lacerated, and he was hospitalized. The forklift was equipped with an operational backup alarm.
