Investigation Summary
At approximately 3:30 p.m. on December 31, 2011, an employee was performing maintenance work on a Ford pick-up truck retro-fitted with a dump truck body while the driver of the vehicle was in the cab of the truck facing away. The employee was working beneath the raised body of the dump, troubleshooting a fuel problem when he inadvertently activated the control switch to lower the dump body. The employee was pinned between the hydraulic pump of the dump truck and the bottom side of the dump bed. The driver of the truck attempted to get to the control switch, but he was unable because the employee's leg was covering it. The driver called emergency services which eventually extricated the employee upon their arrival. The employee was transported to the Lawnwood Regional Hospital, where he was treated, admitted, and place on life support. On January 6, 2012, the employee was removed from life support and at approximately 6:20 p.m., he was pronounced dead.
