Investigation Summary
At approximately 12:30 p.m. on April 13, 2011, Employee #1, a 56-year-old male, was taking a parts inventory from the top platform of a rolling warehouse ladder. The ladder was 30 inches wide and approximately 7 feet, 8 inches tall at the highest point, where Employee #1 was standing. Employee #1 was doing routine work. He worked three to four times a week for approximately four to six hours a day. While standing on the ladder, Employee #1 collapsed, fell, and rolled down to the ground. Employee was taken to the hospital, where he was pronounced dead.
