Investigation Summary

Investigation Nr: 201077484
Event: 09/02/2011
Employee Fractures Leg in Fall

On September 2, 2011, Employee #1 was installing a new bed-liner in the trailer box of a transfer trailer at a quarry and concrete plant. His workshop specialized in the maintenance of a fleet of concrete/sand/gravel delivery trucks. While Employee #1 was tack-welding the new liner to the trailer box, he used a 3-ton weight, connected to an overhead hoist, to hold the floor in position. As he completed the weld and released the weight, the trailer box fell off its chassis and tipped over. Employee #1 and the weight fell from the box to the ground. Employee #1 was taken to SHARP Memorial Hospital, where he was hospitalized and treated for a compound-fracture to the femur bone on one of his legs.
