Investigation Summary

Investigation Nr: 201077013
Event: 08/05/2011
Employee Amputates Thumb on Banding Machine

At approximately 12:00 p.m. on August 5, 2011, Employee #1, a machine operator, was working in a cabinet shop. He was operating the Holzher edge-banding machine (Model Type 1918, Serial Number 1597) that trimmed flat cabinet door pieces and glued a laminate edge on one side. The machine had several blades, including a 4-in. circular blade located within an enclosed housing compartment that was fitted with an interlocked access door. Employee #1 opened the access door to spray the cutting blades with a dry lubricant. After opening the door, the machine's power was shut off, but it had not stopped the rotation of the blades. Employee #1's left hand contacted the 4-in. circular blade, which partially amputated his thumb and lacerated his palm. He was transported to a medical center, where he was treated and released.
