Employee Falls from a Ladder and Fractures Multiple Ribs

At approximately 1:50 p.m., on May 9, 2013, Employee #1 of Welcome Market, Inc. fell half way down from a 10 foot portable ladder. Employee #1 was climbing up the ladder to access foam tray, packaging material at an elevation of approximately nine feet high. The ladder slipped downward. Employee #1 sustained multiple rib fractures. At 2:00 p.m., Employee #1 was transported to the emergency room at the Valley Care Medical Center in Pleasanton. At 6:00 p.m., Employee #1 was then transferred to Eden Medical Center, Castro Valley, admitted, treated and discharged at 9:30 a.m. on May 11, 2013.
