Investigation Summary
On May 7, 2013, Employee #1, a forklift operator of Kerry Inc., was traveling east on the right side of the aisle in the bulk receiving area colloquially known as "Pig Farm" while loaded with empty wooden pallets. Simultaneously, Employee #2, a sanitation employee, was operating/pushing a non-mechanized trash cart towards the west in the opposite direction. As Employee # 1 approached the location where he intended to unload the pallets, he suddenly turned left and collided with Employee # 2's cart, knocking Employee #2 to the ground. Employee #1 sustained a head injury but helped Employee # 2 to get up from the floor. 911 were called and Employee # 2 was transported to Eden medical center where he succumbed to his injuries on May 8, 2014, at approximately 1:45 a.m.
