Worker Injured in Fall from Ladder

At approximately 2:12 p.m. on September 26, 2012, Employee #1, a 52-year-old male with Ssa Terminals, Inc., had just concluded performing preventative maintenance on the trolley rails of intermodal crane X415. Employee #1 stated that he turned himself around to face the ladder when for unknown reasons he slipped and fell. Employee #1 fell approximately ten-feet from the ladder or platform of an intermodal crane to grade. The platform is approximately ten-feet above a grade of asphalt and was approximately five-feet-wide by five-feet-wide. There was a standard guard rail with a toe board, mid-rail and top rail that extended the perimeter of the platform. The platform guard rails did not appear to be damaged or otherwise compromised. No holes or openings were noted in the floor of the platform. No grease or other foreign matter that could have contributed to the incident was observed on the floor of the platform. Employee #1 was taken to the hospital and was hospitalized for four days due to broken left femur. The employer was a stevedoring company with forty-two employees and approximately fifty-acres of property and four intermodal container cranes from the Port of Oakland.
