Employee Collapes in Forklift

On May 17, 2012, Employee #1 was unloading a truck (ID Number 5060). The employee was unstrapping while standing on the floor to release the totes for further unloading by Employee #2, the forklift operator. However, Employee #2 was found lying down, unconscious, bleeding from the back of the head. Employee was admitted to Eden Medical Center, where the employee was treated injury to his head. Employee #2 was released after five days
