Investigation Summary
On May 04, 2012 Employee #1, a service technician of Direct TV, LLC was sent to do service works on an existing satellite dish for a private residential customer. Employee #1 was descending a portable ladder from the roof approximately 11 feet from the composite wood flooring of the porch of the residential house when the ladder collapsed causing him to fall to the floor. Employee #1 sustained a broken ankle and hip requiring surgery. Employee #1 was taken to Oakland Kaiser Hospital by ambulance minutes after the accident.
