Investigation Summary
The accident occurred on November 2, 2013, in a production area, where drugs are packaged. The equipment involved in the accident is called a heat sealer, which seals pills in chambers or blisters of cards. During the period when the heated plate lowered and pressed down, Employee #1's right ring finger became caught under the heated plate for up to eight seconds. A coworker, who heard Employee #1 screaming, went to aid her, and was able to disconnect the compressed air line from the heat sealer to raise the heated plate. Employee #1 was admitted to and discharged from a local hospital on the same day. At the hospital, Employee #1's finger was not amputated, but on November 11, 2011, Employee #1 had her ring finger amputated to and including the first knuckle.
