Investigation Summary

On October 10, 2011, Employee #1, a school secretary, was walking with a large stack of cards from her work area to another room to use an electric hole punch when she fell to the floor. She was hospitalized for a fractured right hip. The interior of the area was well lit with artificial illumination. The floor of the office area was covered in a short-nap carpet that was in excellent condition with no visible tears, runs, seams, bumps, or electrical cords running beneath the carpet. The walking area had no electrical cords, protruding bookcases, shelves, or other obvious slip/trip hazards. Employee #1 could not recall tripping over anything or losing her balance. She did not know the reason for her fall.
