Investigation Summary
On October 19, 2011, Employee #1, of Design Workshops, was cleaning a roller of the Asama Mill without deenergizing the circuit. His right hand was partially drawn in between two rollers, which resulted in a laceration. He was taken to the hospital for treatment and released the same day. At the time of this abstract, an investigation was in process.
