Investigation Summary
On August 24, 2011, an employee was polishing using a portable grinder. The flex wheel broke and struck the employee's left eye thereby causing laceration of the cheek and impact on the eye from the eye goggles. He was released from San Leandro Hospital emergency clinic within three hours.
