Employee's Foot Crushed in Forklift Accident

On Thursday, July 21, 2011 Employee #1, a forklift operator, was operating a forklift and had just finished pulling one order and was heading towards the desk to get another order. As Employee #1 was approaching the desk on the forklift, he could not stop the forklift in time before reaching the bollard just in front of the desk area. When the forklift did stop, it jerked, causing his foot to slip from the foot pedal and out of the operator platform. Employee #1's foot was crushed between the forklift and the bollard. During the incident, Employee #1 panicked, and pulled his foot out of his shoe while it was still pinched between the forklift and bollard. Employee #1 was transported to a local Hospital, where he was hospitalized for five days. Employee #1 sustained severe avulsion and lacerations to his right foot.
