Employee's Hand Is Caught in Machine and Is Amputated

At approximately 11:59 a.m. on May 23, 2011, Employee #1, a senior maintenance technician, was troubleshooting a nonfunctioning conveyor on a panel laminator machine. He was working at a location on the machine where its rollers created a nip point. A coworker was standing outside the saw house building. The building contained a saw unit and a conveyor system with rollers. Employee #1 was inside the saw house adjusting nip rollers. The coworker was on the outside holding a piece of material, which was waiting to be pushed through to the rollers, but then, heard Employee #1 shouting inside the saw house. The coworker went to the control panel and pressed the emergency stop. The conveyor was still running on the outside of the building, where the coworker was located, but the rollers were stopped inside. When the rollers became activated, they pulled Employee #1's right hand into the rollers, causing crush and severe avulsion injuries. Employee #1 was transported to a medical center, where he received treatment and was then released the same day.
