Investigation Summary
On November 16, 2011, Employee #1 was operating a Continental Moss-Gordin cotton gin press (Model Number 730). Employee #2 and #3 were located near the press. As the press was running, there was a compromise in the machine's top seal, which failed because of metal fatigue that caused the collapse of the center column that released two columns that held the seal. The result triggered an explosion that destroyed the press. The top pieces on each of the columns were ejected along with the top seal. The top seal weighed over 6,000 lbs and landed 9 ft from the machine. Employee #1 was struck and killed by the top seal. The sheet metal roof, which was 20 ft above the machine, was visibly damaged when it was struck by one of the ejected columns. Each of the broken columns weighed over 300 lbs. In the explosion, both Employee #2 and #3 were injured. They were transported to a medical center for treatment. Employee #2 received medical care and was released; however, because of the extent of Employee #3's injuries, she not only underwent treatment, but was hospitalized for postoperative care.
