Employee Gets Caught Between Forklift Load and Truck

On November 13, 2010 Employee #1 of Woodard Brothers Corporation was delivering a load of lumber. After lifting the load off the truck, Employee #1 got off the forklift, without applying the brake system, and walked back to the truck. The forklift moved forward pinning him between the truck and the load of lumber. Employee #1 died of asphyxia.
