Investigation Summary
A worker 1 engaged in cleaning out earthen debris washed in form a rain event, out of a partially newly installed 60" inch storm sewer pipe collapsed from health issues. Worker 2 stated to EMS responders the worker 1 had made out of the pipe to the large manhole were a ladder was positioned to egress, started vomiting and collapsed at the foot of the ladder. The Monfort Heights EMS team tested the atmospheric condition of the manhole before entry and found no deficiencies. The EMS team found no vital signs and or body response at 09:37 a.m. that morning. Worker 1 was pronounced deceased by the Mount Airy Hospital at 12:15 p.m. 11/29/2010 with heart failure.
