Investigation Summary
At approximately 3:00 p.m. on April 7, 2011, an employee working in a warehouse was descending a steel rolling warehouse ladder. He fell approximately seven feet onto a concrete floor, suffering a head injury. The Employee was using the ladder to access the conveyor on Machine 70 to push scrap jackets into the feed throat of the grinder. The Employee was taken to the hospital, where he later died.
