Employee Falls from Platform on Forklift

At about 1:00 p.m. on April 15, 2011, an employee received a serious injury to his body while working at his assigned duties of installing windows on a new residence under construction. He and a coworker were about nine feet above the ground on a makeshift wooden platform on the forks of a Gradall forklift. The weight shifted on the platform and caused the single strap to slice through the rear OSB panel, letting the platform to lose stability. The employee was tossed to the ground contributing to his injury. The fire department responded at the scene. The employer reported the injury on April 18, 2011 at 2:30 p.m. An investigation was conducted by the DOSH District Office.
