At approximately 9:00 a.m. on August 25, 2011, Employee #1 was on a catwalk cleaning a conveyor. When he climbed over the conveyor to remove the lid of a tail pulley, he fell approximately 45 ft. Employee #1 was hospitalized for unspecified fractures.
