Investigation Summary
On November 17, 2010, Employee #1 was assisting a truck owner, who was a non-employee, with the closing of the hatch of a truck after it was loaded with a product. Employee #1 used a ladder to reach the hatch. As he closed the hatch, he slipped off the ladder and fell from a height of approximately 7 ft. Employee #1 sustained a fractured arm and leg in the fall. He was transported to a medical center. At the medical center, Employee #1 underwent surgical procedures, which included the stabilizing of the fractured bones with pins and was then hospitalized.
