Investigation Summary
On September 22, 2010, Employee #1 was using a 16 foot ladder to access the roof of a restaurant in order to repair an electrical short on the roof's exhaust system (mounted blower motor). The roof's exhaust system was located 30 feet inward from the roof's edge with no fall hazard present. Employee #1 did not carry a tool box; he only carried the tools necessary for the job, which included wire strippers, a screw driver, and pliers. After repairing the exhaust system, Employee #1 gathered his tools and descended the ladder. However, before he reached the ground, Employee #1 fell and landed on an aboveground water line. The impact of Employee #1's body striking the pipe caused the line to break. The sound of water splashing onto the asphalt alerted a neighbor, who immediately contacted the manager of the restaurant. Emergency services were contacted and, upon arrival, Employee #1 was taken to a nearby trauma center, where he died due to injuries to his head and back. Specifically, Employee #1's spinal cord and aorta were severed, in addition to several broken ribs. No witnesses were present during the accident; however, the accident investigation determined that, due to Employee #1's injuries, he fell backwards, initially striking a two-inch diameter gate frame, and then fell onto the water pipe.
