Investigation Summary
On September 27, 2010, Employee #1 was assisting another employee to move a spot welding machine that weighed approximately 2 tons. Four rollers were placed under the base of the machine to allow it to be pushed to a bay door for loading. When one roller partially out slid from under the machine, Employee #1 pulled it out completely. The machine then began falling and he apparently attempted to stop it. Employee #1 was crushed and killed.
