Employee Dies from Fall While Installing Scaffold

At approximately 1:00 p.m. on December 17, 2010, Employee #1, a coworker and the foreman were installing exterior scaffolding around concrete forms. Employee #1 was working on a single-pole wood scaffold with the platform mounted over brackets built into the concrete forms at a height of approximately 6-7 feet. Employee #1 fell backwards onto a concrete floor below, sustaining lacerations to his head and back. Employee #1 was airlifted to Nassau University Medical Center. Employee #1 died a few weeks later on January 2, 2011.
