Employee's Foot Is Crushed When Run over by Loader

On February 3, 2011, a 34-year-old maintenance assistant employee was working with a crew performing grouting and patching repairs inside a tunnel pipeline. The employee moved, and his left foot was struck by and run over by a loader. The employee underwent surgery for a crushed left foot.
