Investigation Summary

On September 6, 2011 after completing their window washing for the day, Employee #1 and Employee #2 went up to the roof to gather their equipment. Employee #1 told Employee #2 that he wasn't feeling well, and asked if Employee #2 could get his equipment for him. Once Employee #2 got the equipment, he went to check on Employee #1, who he found passed out, on the roof of the tower where they had worked. He called someone, who then contacted hotel security. Hotel security called 911 and performed CPR until paramedics arrived in the Flight for Life helicopter. Employee #1 was airlifted to Spring Valley Hospital Emergency Room, where he was pronounced dead at 6:36 pm. An autopsy report stated: "Cause of Death: It is my opinion that Employee #1 died as a result of CORONORY ATHEROSCLEROSIS. A significant contributing condition is METHAMPHETAMINE INTOXICATION."
