Investigation Summary

On January 26, 2011, Employee #1 and coworkers were unloading a cargo of granite slabs from a trailer mounted container. The first three bundles from the rear of the container had already been removed, and the cradle debris and extra wood had been cleared from the container as Employee # 1 began to cut braces that stabilized the remaining three bundles at the front of the container. He used a DeWalt Sawsall reciprocating saw to cut the wooden braces and stood upon a 4 ft Werner aluminum stepladder to reach them. A fiber-cored wire rope that helped to stabilize the slabs was already cut when he began cutting the braces. He began to cut the braces on the third bundle, but the first two bundles shifted, causing the granite slabs to slide. They struck Employee #1 and pinned him between granite slabs and the third bundle of slabs. He sustained severe trauma injuries to his upper chest. Employee #1 was transported to a medical facility. He remained under medical care until March 31, 2011, when he died.
