Investigation Summary
At approximately 6:00 p.m. on January 20, 2014, Employee #1, a 50-year-old female with Covenant Transport, was standing near the dock area where the trailer was waiting to be hitched to a tractor after it was emptied. The tractor driver was backing toward the front end of the trailer which was parked, unhitched and located at the dock when Employee #1 got between the moving tractor and the parked trailer. Employee #1 was pinned between a Covenant Transport tractor with a sleeper cab and a trailer as the driver was backing up in order to connect the truck to the trailer. Employee #1 suffered crushing injuries to the chest area in the event and required hospitalization for ten days. The width of the trailer was approximately one hundred inches. Covenant Transport was a nationwide trucking company, serving the contiguous 48 states. At the time of the inspection, their headquarters was located in Chattanooga Tennessee.
