Investigation Summary
On November 4, 2012, Employee #1 was working in the workroom of a florist shop. He was standing between two coworkers, when he experienced a medical condition and lost consciousness. Employee #1 rocked forward and fell straight back, striking his head on the floor and sustaining a brain injury.
