Employee Is Crushed by Cut Palm Tree Frond

At approximately 11:00 a.m. on September 22, 2012, Employee #1 was trimming dead leaves off a palm tree. He was at an approximate height of 40 ft. Employee #1 donned climbing gears with rope, spikes and a belt. He then climbed up the palm tree and started cutting dead fronds using a chain saw (STIHL MS 200T). At approximately 12:20 p.m., Emergency service personnel received a call, about him being trapped on the tree. He was located at an approximate height of 30 ft. They reached him and pronounced that he had dead. After clearing the fronds that surrounded him, they brought him down from the tree. A coroner determined the cause of death was a broken neck, which was due to the weight of the dead fronds that struck and crushed him.
