Investigation Summary
On December 12, 2011, Employee #1, a machine operator, was operating a fusing machine; a belt loop cutting machine equipped with a heat transfer press. The machine is used to weave fabric and then cut the fabric into 1 in. by 3 in. pieces. Fabric pieces jammed in the chute of the machine. The machine was not deenergized and a guard was not installed. Employee #1 stuck his left hand's index finger into the chute to dislodge the fabric. Employee #1 sustained an amputation of the distal phalanx of the left index finger. Employee #1 was not hospitalized. The Employer was cited for two Serious Accident Related violation set forth in Title 8 California Code of Regulation Section: 3314 C -The Control of Hazardous Energy for the Cleaning, Repairing, Servicing, Setting-Up, and Adjusting Operations of Prime Movers, Machinery and Equipment, Including Lockout/Tagout; and, 4186 B - Maintenance and Use of Point of Operation Tools and Guards.
