Employee Amputates Finger on Chain and Sprocket Unit

At approximately 7:00 p.m. on September 29, 2011, Employee #1, an operating engineer, was working alone when he replaced the chain and sprocket on the traffic control unit of the Swisslog pneumatic tube system. As he was attempting to replace the cover, his right middle finger tip was amputated after becoming trapped between the chain and sprocket of the unit. Employee #1 stated that he had deenergized the equipment to replace the chain and sprocket but had reenergized the equipment to check the alignment. However, he inadvertently deenergized the wrong traffic control unit. The unit he was working on was energized at the time. He was not hospitalized for his injury.
