Investigation Summary
At approximately 10:10 a.m. on June 11, 2011, Employee #1 was clearing a jammed wood slave board inside warehouse 417. Employee #1 picked up a jammed slave board with his right hand, blinding one of the sensors, triggering another slave board to move. This action pinned Employee #1's hand against the frame of the spur machine. Employee #1 was taken to the hospital and treated for lacerations of two of his fingers.
