Employee Is Injured in Fall in Excavation

Approximately 11:06 a.m. on July 7, 2011, Employee #1 attempted to enter a 4 feet deep, 5 feet wide and 8 feet long excavation to make a measurement for pipes installation. The excavation was protected by 2 x 4 guardrails except on one side that was sloped 1:1 for employees(s) to enter into the excavation. Employee #1 fell into the excavation due to loose gravel and hit the edge of a parked excavator basket. Employee # 1 sustained serious internal injuries. He was transported to UCLA Medical Center, where he was hospitalized for seven days. The accident was reported to the DOSH (Division of Occupational Safety and Health) on July7, 2011.
